i highlight the common drug dosing for common tdm request

vancomycin: max 10mg/kg/dose for every 8-12hours
prematured baby on gentamicin: 3.5mg/kg/dose OD
normal baby on gentamicin: 5mg/kg/dose OD
amikacin: max maintenance dose 7.5mg/kg/dose OD
sodium valproate: max 20mg/kg/dose every 12hours..
